ABSTRACT

La ingesta de cuerpos extraños es una situación frecuente y cotidiana. La mayoría de las veces cursa con resolución espontánea, pero cuando el cuerpo extraño resulta ser un elemento punzante, suele producir perforación esofágica con el consiguiente riesgo de mediastinitis y/o sangrado. Se presenta un caso crítico derivado de diferentes centros asistenciales donde se logra mediante un manejo y tratamiento multidisciplinario, un resultado favorable


The intake of foreign bodies is a frequent and daily situation. Most of the time he has spontaneous resolution, but when the foreign body turns out to be a sharp element, it usually causes esophageal drilling with the consequent risk of mediastinitis and/or bleeding. ABSTRACT

Las enfermedades obstructivas de la vía aérea pediátrica son muy frecuentes debido a los fenómenos mecánicos que están involucrados. En los niños más pequeños, la marcada resistencia de las vías aéreas pequeñas, determinada por la falta de tejido elástico y una caja torácica aún no bien desarrollada; tanto su estructura como la musculatura, facilitarán que cuadros infecciosos, mecánicos (cuerpo extraño) y compresivos, determinen que los flujos de aire se vean limitados y con ello la ventilación alveolar. La respuesta fisiológica con aumento del trabajo respiratorio es limitada y por lo tanto la fatiga muscular determinará hipoventilación con las consecuencias de hipoxemia e hipercapnia.


Obstructive diseases of the pediatric airway are very frequent due to the mechanical phenomena that are involved. The marked resistance of the small airways, such as the lack of elastic tissue and a thoracic cage that is not yet well developed, both in its structure and in the musculature, will make it easier for infectious, mechanical (foreign body), compressive and other conditions to determine that the flows of air are limited and with it the alveolar ventilation. The physiological response with increased work of breathing is limited and therefore muscle fatigue will determine hypoventilation, with the consequences of hypoxemia and hypercapnia.

ABSTRACT

El pegamento basado en cianoacrilato posee una gran capacidad de adherencia a los tejidos, representando un problema cuando se encuentra en el oído externo debido a sus características anatómicas particulares. Se presenta un caso clínico de cuerpo extraño de cianoacrilato que ocluye el conducto auditivo externo y el tímpano, alterando la audición. Se describen los hallazgos y los intentos de extracción utilizando las alternativas terapéuticas descritas en la literatura, sin obtener resultados positivos, debiendo recurrir a la extracción quirúrgica. Además, se presentan detalles del procedimiento y los resultados. El paciente recupera la audición y la normalidad anatómica.


Cyanoacrylate-based glue has a great capacity for adhering to tissue, which is a problem when it is placed in the ear canal due to the anatomy of this structure. A clinical case of a cyanoacrylate foreign body occluding the external auditory canal and the tympanic membrane is presented. The therapeutics alternatives described in the literature used in the case failed, so, it was surgically removed by drilling the glue. Details of the procedure and results are presented. The patient recovers the hearing and anatomical normality.

ABSTRACT

Objective: To analyze the clinical characteristics and complications of esophageal foreign bodies of button battery ingestion in children. Methods: A retrospective descriptive study included 83 children who were hospitalized in our hospital on account of button battery ingestion from January 2011 to December 2021. There were 50 males (60.2%) and 33 females (39.8%). The age ranged from 7.6 months to one month off 10 years, with a median age of 18 months. The data of patient demographics and time from ingestion to admission, location, symptoms, management, complications, and follow-up outcome were recorded. SPSS17.0 software was used for statistical analysis. Results: Seventy-two children (86.7%) were younger than 3 years old. The time from ingestion to admission ranged from 1 h to 2 months, with a median time of 8 h. Among the 63 children who were first diagnosed in our hospital, the most common clinical symptoms were nausea and vomiting (32 cases, 50.8%), dysphagia (31 cases, 49.2%), salivation (11 cases, 17.5%) and fever (10 cases, 15.9%). Seventy-three of 83 cases had complete preoperative diagnostic tests, and 55 cases (75.3%) were diagnosed by X-ray. In 56 cases (76.7%), the foreign badies were impacted in the upper third of esophagus. In 72 cases (86.7%), the foreign badies were removed by rigid esophagoscopy. 23 (27.7%) had serious complications, including tracheoesophageal fistula in 15 cases(TEF;65.2%), vocal cord paralysis (VCP;34.8%) in 8 cases, esophageal perforation in 3 cases (EP;13.0%), hemorrhage in 3 cases(13.0%), mediastinitis in 3 cases (13%), and periesophageal abscess in 1 case (4.3%). There were significant differences in the exposure time of foreign bodies and unwitnessed ingestion by guardians in the complications group (P<0.05). 2 cases died (2.4%)respectively due to arterial esophageal fistula bleeding and respiratory failure caused by stent displacement during the treatment of tracheoesophageal fistula. Conclusion: Accidental button battery ingestion can be life-threatening. and it mostly happens in children under 3 years old. Serious complications may happen cause of non-specific clinical manifestations and unwitnessed ingestions. Anterior and lateral chest X-ray is the first examination choice. Tracheoesophageal fistula is the most common serious complication.

ABSTRACT

Foreign body reactions induced by macrophages often cause delay or failure of wound healing in the application of tissue engineering scaffolds. This study explores the application of nanosilver (NAg) to reduce foreign body reactions during scaffold transplantation. An NAg hybrid collagen-chitosan scaffold (NAg-CCS) was prepared using the freeze-drying method. The NAg-CCS was implanted on the back of rats to evaluate the effects on foreign body reactions. Skin tissue samples were collected for histological and immunological evaluation at variable intervals. Miniature pigs were used to assess the effects of NAg on skin wound healing. The wounds were photographed, and tissue samples were collected for molecular biological analysis at different time points post-transplantation. NAg-CCS has a porous structure and the results showed that it could release NAg constantly for two weeks. The NAg-CCS group rarely developed a foreign body reaction, while the blank-CCS group showed granulomas or necrosis in the subcutaneous grafting experiment. Both matrix metalloproteinase-1 (MMP-1) and tissue inhibitor of metalloproteinase-1 (TIMP-1) were reduced significantly in the NAg-CCS group. The NAg-CCS group had higher interleukin (IL)-10 and lower IL-6 than the blank CCS group. In the wound healing study, M1 macrophage activation and inflammatory-related proteins (inducible nitric oxide synthase (iNOS), IL-6, and interferon-‍γ (IFN-‍γ)) were inhibited by NAg. In contrast, M2 macrophage activation and proinflammatory proteins (arginase-1, major histocompatibility complex-II (MHC-II), and found in inflammatory zone-1 (FIZZ-1)) were promoted, and this was responsible for suppressing the foreign body responses and accelerating wound healing. In conclusion, dermal scaffolds containing NAg suppressed the foreign body reaction by regulating macrophages and the expression of inflammatory cytokines, thereby promoting wound healing.

ABSTRACT

Chest pain is one of the most common complaints in the emergency department. Diseases of the heart, aorta, lungs, esophagus, stomach, mediastinum, pleura, and abdominal viscera can all cause chest discomfort (Gulati et al., 2021; Jiao et al., 2021; Lu et al., 2022). Clinicians in the emergency department are expected to immediately recognize life-threatening chest pain (Jiao et al., 2021). Delayed diagnosis further increases the risk of complications and mortality (Liu et al., 2021). In this case, we present an elderly Chinese female who had a history of myocardial infarction two years previously, with chest pain eventually found to be caused by ingestion of a duck bone.

RESUMEN Los cuerpos extraños en el esófago son un problema frecuente en urgencias, debido la mayoría de las veces a ingestión accidental. Presentamos el caso de una paciente femenina de 50 años, sin antecedentes patológicos, que consulta por presentar la ingesta de prótesis dental mientras almorzaba, con posterior disfagia, odinofagia y dolor cervical. Se le realizan radiografías y tomografía computarizada (TC) donde se evidencia imagen radiopaca en esófago cervical. Se interconsulta con el Servicio de Gastroenterología, dada la imposibilidad de extracción de la prótesis por vía endoscópica. Se realiza luego cervicotomía lateral izquierda de emergencia, con esofagotomía, extracción del cuerpo extraño y cierre primario del esófago. La obstrucción del esófago por cuerpos extraños puede ocasionar muchas complicaciones. La consecuente demora en el tratamiento puede llevar a un marcado aumento de la mortalidad. Por eso, en este caso, se prioriza la cirugía ante un riesgo elevado de perforación y hemorragia.


ABSTRACT Esophageal foreign bodies are a common problem in the emergency department, usually due to accidental ingestion. We report the case of a 50-year-old otherwise healthy female patient who sought medical care due to dysphagia, odynophagia and cervical pain after swallowing a partial denture while having lunch. The patient underwent X-rays and a computed tomography (CT) scan which showed a radiopaque image in the upper esophagus. After a failed attempt to remove the denture through endoscopy, surgery was decided via a left lateral emergency cervicotomy with esophagotomy, foreign body removal and primary closure of the esophagus. Esophageal obstruction due to foreign bodies may cause many complications. Treatment delays may result in increased mortality. Thus, in this case we prioritized surgery due to high risk of perforation and bleeding.

ABSTRACT

Impalement injuries are a complex and rare type of penetrating abdominal trauma that happens when an object such as a post or a pole penetrates a person injuring several organs, making it a life-threatening situation in which time and correct management play an important part in the survival of the patient. A 37-year-old man suffered abdominal impalement injury with a metal signal post, penetrating the left flank of the abdomen. On examination, there is a hypoventilated left hemithorax with intercostal retractions, increased heart rate, weak distal pulses, delayed capillary refill, and pale skin. A 1-meter-long metal post (approximately 7cm diameter) penetrates the left flank with the entry in the posterior lumbar region. Abdominal viscera, omentum, intestinal content, and ischemic loops of the small intestine are visible. An exploratory laparotomy was performed; left hemicolectomy, end colostomy and Hartmann procedure, resection of the affected jejunum, and end-to-end anastomosis were performed. On the ninth postoperative day, an abdominal tomography was performed due to the presence of fever peaks, which reported thrombosis of the left renal artery and emphysematous pyelonephritis, with the presence of a left pararenal collection. A simple left nephrectomy was performed. Postoperative surveillance was satisfactory during the following 5 days. The patient was discharged. An impaled injury is a complex lesion that needs special attention from the medical field for correct management. Although there is some literature about it, we encourage more research to be done about impalement injuries.


Las lesiones por empalamiento son un tipo de traumatismo abdominal penetrante complejo y raro de que se produce cuando un objeto, como un poste o una vara, penetra a una persona lesionando varios órganos, lo que la convierte en una situación potencialmente mortal en la que el tiempo y el manejo correcto juegan un papel importante en la supervivencia del paciente. Un hombre de 37 años sufrió una herida por empalamiento abdominal con un poste de señales de metal, penetrando el flanco izquierdo del abdomen. A la exploración física, hay un hemitórax izquierdo hipoventilado con retracciones intercostales, aumento de la frecuencia cardíaca, pulsos distales débiles, relleno capilar retrasado y piel pálida. Un poste metálico de 1 metro de largo (aproximadamente 7 cm de diámetro) penetra el flanco izquierdo con entrada en la región lumbar posterior. Son visibles las vísceras abdominales, el epiplón, el contenido intestinal y las asas isquémicas del intestino delgado. Se realizó una laparotomía exploradora; Se realizó hemicolectomía izquierda, colostomía terminal y procedimiento de Hartmann, resección del yeyuno afectado y anastomosis terminoterminal. Al noveno día postoperatorio se realiza tomografía abdominal por presencia de picos febriles, que reporta trombosis de arteria renal izquierda y pielonefritis enfisematosa, con presencia de colección pararrenal izquierda. Se realizó nefrectomía izquierda simple. La vigilancia postoperatoria fue satisfactoria durante los siguientes 5 días. El paciente fue dado de alta. Una lesión por empalamiento es una lesión compleja que necesita una atención especial desde el ámbito médico para su correcto manejo. Aunque existe cierta literatura al respecto, alentamos a que se realicen más investigaciones sobre estas lesiones.

Introduction: Body piercings consist of small holes made with a needle in different parts of the skin or body to introduce a jewel or decorative element. In the oral cavity, most piercings are placed in the tongue. However, some complications may occur, and surgical techniques must be used for their removal. These complications present a certain degree of difficulty due to their position and may challenge the ability of the clinician to access the specific anatomical location. The different imaging techniques, from simple radiography to intraoperative techniques such as image intensifiers, have become an extremely useful tool for locating an object in the three dimensions of space, allowing safe location and extraction. Objective: The aim of this study is to report the case of a complication of a body piercing in the oral cavity and how the use of imaging was decisive for surgical planning and for the quick and effective resolution of the case. Material and Methods: A 14-year-old female patient came looking for treatment. Her mother reported the onset of the condition after the insertion of a needle-like metallic object while performing an artistic perforation in the lingual region. Since the girl was unable to extract the object, she sought medical advice at the Carlos Arvelo Military Hospital in Caracas, Venezuela. Subsequently, an imaging study was performed by means of a Computed Tomography to locate the metallic object. It was observed that the foreign body had migrated to the floor of the mouth/sublingual region, requiring the area to be surgically approached. It was also decided to use an intraoperative image intensifier. The removal of the object was performed satisfactorily. Conclusion: The extraction of foreign bodies placed in the lingual and sublingual region represents a challenge for the clinician due to the number of important anatomical structures that pass through that area. This makes clinicians plan their surgical removal using pre- and intraoperative imaging, to find a less traumatic location, reduce surgical time as well as the risk of damaging adjacent anatomical structures.


Introducción: Los body piercings consisten en producir perforaciones con una aguja en diferentes localizaciones de la piel con el fin de introducir una joya o elemento decorativo a través del agujero producido previamente. En la cavidad oral, la lengua es el sitio de mayor elección; sin embargo, en ciertas ocasiones suelen presentarse ciertas complicaciones, por lo cual se debe recurrir a técnicas quirúrgicas para su remoción presentando cierto grado de dificultad para su localización y la capacidad del clínico para acceder al espacio anatómico. Las diferentes técnicas imagenológicas desde una radiografía simple hasta técnicas intraoperatorias como los intensificadores de imágenes se han convertido en una herramienta sumamente útil para la ubicación de un objeto en las tres dimensiones del espacio, permitiendo una localización y extracción segura para el clínico y el paciente. Objetivo: El propósito de este trabajo es reportar un caso de una complicación de esta práctica en la región bucal y cómo el uso de la imagenología fue determinante para la planificación quirúrgica y la resolución del caso de manera rápida y efectiva. Material y Métodos: Se presenta a consulta una paciente femenina de 14 años de edad, quien madre refiere inicio de enfermedad actual posterior a introducirse objeto metálico tipo aguja de compás con la finalidad de realizar perforación artística en región lingual, al no poder extraer dicho objeto, acude a evaluación en el Hospital Militar Carlos arvelo de Caracas; posterior se realiza estudio imagenológico tipo Tomografía Computarizada para la ubicación del objeto metálico donde se observa que el cuerpo extraño habría migrado hacia el piso de boca/región sublingual, teniendo la necesidad de abordar quirúrgicamente la zona y elegir el uso de intensificador de imagen transoperatorio para retiro del mismo el cual se dio de manera satisfactoria. Conclusión: Los cuerpos extraños desplazados a la región lingual y sublingual representan un desafío para el clínico al momento de extraerlos, esto se debe a la cantidad de estructuras anatómicas importantes que pasan por dicha zona, lo que hace que el clínico opte en su planificación quirúrgica por el uso de imagenología pre y transoperatoria obteniendo así una localización menos traumática, menor tiempo operatorio y menor riesgo de lesión de estructuras anatómicas adyacentes.

Resumen Introducción: Los cuerpos extraños (CE) en oído, nariz y vía aéreodigestiva superior son el 30% de las urgencia en otorrinolaringología (ORL). No existen datos epidemiológicos nacionales que describan la casuística de CE. Objetivo: Describir la epidemiología de pacientes con diagnóstico de CE evaluados, entre el 2013-2018 en Clínica Santa María de Santiago, Chile. Describir las características de los CE, ubicación, método de extracción y complicaciones. Material y Método: Estudio descriptivo y retrospectivo, basado en revisión de fichas clínicas de pacientes con diagnóstico de CE, analizando variables epidemiológicas y clínicas. Resultados: Se revisaron 1.847 casos con diagnóstico de CE, confirmando 1.494. La mayoría de sexo masculino (53,3%), con un promedio de edad de 16,5 años (rango de 0-95 años). Los CE más prevalentes fueron ótico (52,9%) y nasal (27,9%). El síntoma asociado más frecuente fue la sensación de CE (18,1%). El diagnóstico fue principalmente por examen físico (84%), requiriéndose exámenes complementarios en 18,2%. La extracción fue ambulatoria en su mayoría (84,6%), requiriendo extracción en pabellón el 12,5%. El 5,5% presentó complicaciones, y la tasa de letalidad fue de 0,07%. Conclusión: Esta casuística, única a nivel nacional, nos permite conocer la epidemiología de los CE. Un bajo porcentaje de pacientes fue de riesgo vital, pero el manejo oportuno permite una baja tasa de complicaciones y letalidad.


Abstract Introduction: Foreign bodies (FB) in the ear, nose and upper airway-digestive tract are 30% of the emergencies in otolaryngology (ORL). There is no national epidemiological data that describes the FB casuistry. Aim: To describe the epidemiology of patients who have a diagnosis of a FB evaluated, between 2013-2018, at Santa María Clinic in Santiago, Chile. To describe the characteristics of FB, location, method of removal, and complications. Material and Method: A descriptive and retrospective study, based on review of clinical records of patients with the diagnosis of FB, analyzing epidemiological and clinical variables. Results: 1847 cases with a diagnosis of FB were reviewed, of which 1494 were confirmed. Most were male (53.3%), with an average age of 16.5 years (range 0-95 years). The most prevalent FB locations were otic (52.9%) and nasal (27.9%). The most frequent associated symptom was sensation of a FB (18.1%). The diagnosis was fundamentally based on physical examination (84%), requiring complementary tools in 18.2%. The extraction was mostly ambulatory (84.6%), requiring extraction in the operation room in 12.5%. 5.5% presented complications, and the fatality rate was 0.07%. Conclusion: This casuistry is unique at the national level, and allows us to know the epidemiology of FB. A low percentage of patients correspond to life-threatening locations, but timely management allows a low rate of complications and fatality.

El sangrado genital en niñas prepúberes es un signo poco frecuente y siempre requiere determinar su causa. Se necesitan una detallada anamnesis y examen físico, con el conocimiento adecuado de la anatomía uroginecológica, y, en muchos casos, estudios de imágenes y exámenes complementarios, para arribar al diagnóstico. Se presenta el caso de una niña de 7 años con sangrado genital, cuyo examen físico y estudios complementarios fueron poco concluyentes, y que requirió un procedimiento invasivo para su resolución. Genital bleeding in prepubertal girls is a rare sign and always requires determining its cause. A detailed history and physical examination are needed, with adequate knowledge of urogynecological anatomy, and in many cases, imaging studies and complementary tests, to arrive at the diagnosis. We present the case of a 7-year-old girl with genital bleeding, whose physical examination and complementary studies were inconclusive, requiring an invasive procedure for its resolution. (AU)

ABSTRACT

Introducción: el edema pulmonar posobstructivo (EPPO), o por presión negativa, es una entidad potencialmente mortal, que se desarrolla inmediatamente luego de una obstrucción severa de la vía aérea superior. Materiales y métodos: descripción de una serie de 4 casos de EPPO ocurridos en niños, 3 de ellos secundarios a aspiración de un cuerpo extraño y el otro como complicación de una adenoamigdalectomía. Discusión: la causa más frecuente de la obstrucción de la vía aérea es el laringoespasmo asociado con la manipulación de la vía aérea durante la intubación o las intervenciones quirúrgicas de la vía aérea. Tanto la adenoamigdalectomía, como la extracción de cuerpos extraños en la vía aérea constituyen unas de las intervenciones más frecuentes de la práctica otorrinolaringológica para el tratamiento de la obstrucción de la vía aérea; sin embargo, puede potencialmente desarrollar EPPO. Conclusión: Destacamos la importancia de que el otorrinolaringólogo tenga presente esta afección en niños que presentan dificultad respiratoria tras cualquier obstrucción o intervención quirúrgica de la vía aérea.si bien los cuerpos extraños en la vía aérea en niños suelen presentarse con crisis de asfixia, tos paroxística o dificultad respiratoria luego del evento, también debería pensarse la posibilidad de un evento de aspiración de un cuerpo extraño no presenciado ante un cuadro de edema pulmonar sin causa conocida. Si bien la adenoamigdalectomía es una de las cirugías más frecuente en la práctica otorrinolaringológica, esta potencialmente puede complicarse con EPPO.


Introduction: Post-obstructive pulmonary edema (POPE) or by Negative Pressure, is a potentially fatal entity that develops immediately after a severe obstruction of the upper airway. Materials and methods: Description of a series of four cases of POPE in children, three of them secondary to foreign body aspiration and the remaining one as a complication of adenotonsillectomy. Discussion: The most common etiology of airway obstruction is laryngospasm associated to airway manipulation during intubation or airway surgery. Both adenotonsillectomy and removal of foreign bodies in the airway are one of the most common procedures in otorhinolaryngology practice for management of airway obstruction, however, they can potentially develop EPPO. Conclusion: Although airway foreign bodies in children usually present with sudden episode of choking, paroxysmal cough and/or respiratory distress, the likelihood of an unwitnessed foreign body aspiration event in the presence of unexplained pulmonary edema should also be considered. Although adenotonsillectomy is one of the most common surgeries in ENT practice, it can potentially be complicated by EPPO. We emphasize the importance of the otorhinolaryngologist keeping this condition in mind in children who present respiratory distress after any obstruction or surgical intervention of the airway.

La extracción artroscópica de un proyectil articular de cadera por herida de bala se introdujo por primera vez por Cory y cols. en 1998. El misil puede ubicarse flotando libremente dentro de la articulación, ubicado cercano a esta o alojado dentro del hueso del acetábulo o de la cabeza femoral. Se presenta el caso de un paciente de treinta y dos años, quien fue asaltado. El proyectil se alojaba en el trasfondo del acetábulo luego de un trayecto abdominal.La artroscopía de cadera es un procedimiento mínimamente invasivo que resultó efectivo para el retiro de un proyectil articular, resultando menos cruento que una artrotomía. No se encontraron complicaciones. Nivel de Evidencia: III


Arthroscopic removal of a bullet from the hip joint was introduced first by Cory et al. in 1998. The missile can be located freely floating within the joint, located close to the joint, or lodged within the acetabulum bone or the femoral head. A case is presented in a thirty-two-year-old patient, who was assaulted. The projectile lodged in the background of acetabula fossa after an abdominal trajectory.Hip arthroscopy is a minimally invasive procedure that was effective for the removal of a joint projectile resulting in less morbidity than an arthrotomy. No complications were found. Level of Evidence: III

In ecological parks, the proximity to tourist activities facilitates the exploration of garbage by coatis, with possible serious consequences for the animals health . We described the contents of wild coatis feces from three ecological parks. After analyzing 62 samples, fragments of plants and animals were identified in all feces. In the feces of two parks, seeds were present between 36.4% and 48.6% of the samples. Arthropod fragments were identified in 100% of the samples from two parks, but only 87.3% in a third park. Scales, bones or bird feathers were present in some samples. Undigested material of industrial origin was detected in 34.3% to 54.5% of the samples, such as fragments of paper, string, plastic, aluminum, latex and glass. Results are in line with other studies on the diet of wild coatis, but the intake of foreign bodies, potentially harmful to health, is described for the first time. Clinical problems resulting from ingesting waste can be dental fractures, mucosal erosions, intestinal perforation, peritonitis, impaction, diarrhea, weight loss, intoxication and infections. Coatis in the three parks are at risk of health, and actions are needed to avoid clinical and potentially fatal problems. Four actions are recommended to avoid ingesting foreign bodies: increasing the environmental education of visitors; improving the storage of waste generated in parks; periodically monitor the health of coatis, in order to make interventions when possible; make a permanent program to study the ecology of species in the three parks.


Em parques ecológicos, a proximidade com atividades turísticas facilita a exploração do lixo por quatis (Nasua nasua), com possíveis consequências graves para saúde dos animais. Descrevemos o conteúdo de fezes de quatis selvagens de três parques ecológicos. Após análise de 62 amostras, fragmentos de plantas e animais foram identificados em todas as fezes. Nas fezes de dois parques, as sementes estiveram presentes entre 36,4% e 48,6% das amostras. Foram identificados fragmentos de artrópodes em 100% das amostras de dois parques, mas apenas 87,3% em um terceiro parque. Escamas, ossos ou penas de pássaros estavam presentes em algumas amostras. Detectou-se material não digerido de origem industrial em 34,3% a 54,5% das amostras, como fragmentos de papel, barbante, plástico, alumínio, látex e vidro. Os resultados estão de acordo com outros estudos sobre a dieta de quatis selvagens, mas a ingestão de corpos estranhos, potencialmente prejudicial à saúde, é descrito pela primeira vez. Os problemas clínicos decorrentes da ingestão de lixo podem ser fraturas dentais, erosões de mucosas, perfuração intestinal, peritonite, impactação, diarreia, emagrecimento, intoxicação e infecções. Os quatis nos três parques estão com a saúde em risco, sendo necessárias ações para evitar problemas clínicos e potencialmente fatais. Quatro ações são recomendadas para evitar a ingestão de corpos estranhos: aumentar a educação ambiental dos visitantes; melhorar o armazenamento dos resíduos gerados nos parques; monitorar periodicamente a saúde dos quatis, de forma a fazer intervenções quando possível; fazer um programa permanente de estudo da ecologia das espécies nos três parques.

ABSTRACT

A 59-year-old male patient with local sinus tract formation due to residual foreign body was admitted to the Second Affiliated Hospital of Zhejiang University College of Medicine on December 17, 2018. The examination showed that the residual foreign body was the component of a sticky cloth implanted when the patient underwent appendectomy 27 years ago. Hypertrophic scar developed at the right-lower abdominal incision for appendectomy 23 years ago and the secondary infection after cicatrectomy resulted in non-healing of the wound. The chronic refractory wound healed completely after surgical treatment in our hospital after this admission. The postoperative pathological examination revealed local inflammatory granuloma. This case suggests that chronic refractory wound is likely to form when secondary infection occurs following the surgical procedure near the implant, and aggressive surgery is an effective way to solve this problem.

INTRODUCTION@#Aural foreign bodies (FBs) are a common presenting complaint in emergency departments (EDs) worldwide. This study aims to describe trends and outcomes of aural FBs in the paediatric population, presenting to a tertiary hospital in Singapore.@*METHODS@#A retrospective review of medical records was conducted of all children 0-16 years old with aural FBs who presented to KK Women's and Children's Hospital ED from 2013 to 2017. Clinical data that were collected include patient demographics, type of FB, ear compartment and laterality of FB, symptoms, duration of impaction, mode of removal, outcome in ED, and final disposition.@*RESULTS@#There were a total of 1,003 cases. The largest age group consisted of 53.7% preschool children of 0-6 years. Males (61.7%) were more common than females (38.3%). FBs were predominantly organic materials (25.6%), followed by beads and stones (15.2%). Most FBs were found in the right ear (56.6%). The majority of patients were asymptomatic (62%). Symptoms observed included ear pain (20.1%), itch (4.8%) and bleeding (3.2%). FBs were removed by instruments (36.6%), suctioning (15.4%), syringing (8.2%), or a combination of methods (13.7%). In the ED, 73.9% of patients had an attempt at removal, among which 78.4% of FBs were successfully removed, 5.9% required specialist review, and 15.7% were unsuccessful.@*CONCLUSION@#The majority of paediatric aural FBs can be successfully removed in the ED. Emergency physicians should be trained and equipped with the relevant skills to remove aural FBs.

ABSTRACT

ABSTRACT Migration of foreign bodies into the urinary tract is a rare event. In certain instances, to unravel the way that objects arrived in the urinary tract is not easy. We report the case of an accidentally swallowed wooden toothpick that migrated and was found in the left ureterovesical junction, protruding into the bladder. Even though the computed tomography scan is widely employed to evaluate the urinary tract, this resource does not have a good sensitivity for detecting foreign bodies. Our report presents an insight into the best imaging approach if wooden toothpicks are suspected. In the present case, the endoscopic treatment was possible with an uneventful outcome and a complete resolution of symptoms.
